Cancellation & No-Show Policy
Our staff value your time and have reserved a time slot for your vision and eye health concerns to be addressed by one of our optometrists. We ask that you provide our office with at least 24 hours’ notice for any cancelled or rescheduled appointment. If less than 24 hours’ notice is given each cancelled or rescheduled or missed appointment will be subject to a $50.00 fee. This fee will still apply if you cancel due to insurance not being active, available, or out-of-network. Please notify our office with at least 24 hours’ notice by calling us at (970)667-1866.
We understand situations arise in which you may need to cancel your appointment. If you must change your appointment, please call us at least 24-hours in advance. Advance notice will allow other patients in need of eye health and vision services the opportunity to be seen in the allotted time that was originally set aside for you.
Two or more no-shows or cancellations in a one-year period may be cause for dismissal from our practice. We greatly appreciate your understanding and cooperation with this policy.
